It's been confirmed by Valve that Doug was dead a long time before Portal 2, I assume that he bled to death after falling asleep in the relaxation whateveritis. Also, it's not rambling. There are several possible translations of what he's really saying.
But isn't it creepy as hell that you can hear the dead guy speaking? And that no one knows who voiced him?
I think Chell really did have brain damage.

The official Aperture Science Collaborative Disposition Test describes Doug Rattmann as:
